Interactive Voice Response (IVR) based health information for antenatal and postnatal care services in rural Bangladesh

Background:
Despite significant improvements in maternal and child health over the last three decades, Bangladesh's maternal mortality rate remains alarmingly high at 176 per 100,000 live births (UNICEF 2021). Neonatal deaths account for 67% of all under-5 deaths in Bangladesh (BDHS 2020), with a high risk of mortality in the first month of life. Key challenges include limited access to quality maternal health care, low health literacy, traditional gender norms, and cultural beliefs about pregnancy, particularly in rural areas.


Objectives:
This study aims to use mobile technology to provide maternal health information through an Interactive Voice Response (IVR) system and a virtual call center. The goal is to increase the time and quality of maternal healthcare services during the antenatal and postnatal periods, ultimately reducing maternal and neonatal mortality rates.


Methodology:
A village-level clustered randomized controlled trial (RCT) will be conducted, involving 220 villages in rural Bangladesh. The study will include approximately 2,400 pregnant women in the treatment group and 600 "untreated" women to assess potential spillover effects. Participants will receive IVR-based modules and SMS reminders every two weeks, covering pregnancy-specific information and postnatal care guidelines. An advisory service will also be available via a toll-free call center.
